Application Conditions and Instructions:

• Applications of all candidates will be accepted online only. Applications received through any medium other than online will be treated as outright cancellation. The candidate’s signature must be identical in the application form, written, oral and practical (where applicable) examinations.
• The latest rules and regulations of the government will be followed in the matter of recruitment and quota determination.
• Candidate should be a citizen of Bangladesh by birth and a permanent resident of Lakshmipur district. If a candidate is not a citizen of Bangladesh or married or committed to marry a person who is not a citizen of Bangladesh, he/she will not be considered eligible to apply.
• If a candidate has been convicted by a court of a criminal offense or has been dismissed from the employment of any Government or Autonomous Institution or Local Authority, he will not be considered eligible to apply.
• Candidate Age 22/04/2024 AD Minimum 18 (eighteen) and maximum 30 (thirty) years as on date. However, the maximum age limit is 32 (thirty two) years in case of children of brave freedom fighters / martyred brave freedom fighters and physically challenged. Affidavit is not admissible for proof of age.
• Candidates can apply for only one post. If applying for more than one post, all his applications will be considered cancelled.
• The applicant should mention all his acquired educational qualifications / experience in the application.
• Department of Health Memorandum No. Swa: Adhi/Prasha-2/3rd Grade Recruitment-3/2018/4525 Dated: 08/10/2018 AD. As per the originally published notification those who have previously applied online for Statistician and Office Assistant Cum Computer Numerical Posts need not reapply. Their application will be admissible if correct and their age will be reckonable as per the conditions of previous recruitment notification. Admit cards will be issued to eligible candidates from among them to participate in the recruitment test based on the currently issued recruitment circular.
• Candidates for Government / Semi-Government / Autonomous / Semi-Autonomous Organizations must apply within prescribed time limits following the Government Rules and all the candidates must submit the original copy of No Objection Certificate (NOC) issued by the appointing authority at the time of oral examination. .
• If the Permanent Address mentioned by the candidate in the application form is different from the permanent address mentioned earlier in any certificate or elsewhere or if the address of the husband is used in the case of female candidates, then the candidate is required to give the changed permanent address National Identity Card (NID) and Citizenship Certificate signed by concerned Union Parishad Chairman / Municipal Mayor should be submitted in favor of address.
• In case of Health Assistant post, the candidate should be a permanent resident of the ward (old) for which he/she is applying for the post. (Old) permanent residents of other wards of the same union can also apply, in which case the ward in which the candidate is a resident must be mentioned in the application form. However, if eligible candidates resident of the respective vacant post are found, candidates from other wards will not be considered and if eligible candidates resident of the respective ward are not available, eligible candidates from other wards will be considered. Application in wrong ward will be considered as rejected.
• Vacancy list of Health Assistant (Ward/Union/Upazila wise) is available on notice board of Office of Civil Surgeon, (Lakshmipur Civil Surgeon Office Job Circular) and all Upazila Health and Family Planning Officers under its jurisdiction and on website (http://cslakshmipur.teletalk.com.bd).
• Candidates who qualify the written test for the posts of Statistician, Store Keeper and Health Assistant will be admitted to the oral test. However, candidates who have cleared the written test for the post of Office Assistant cum Computer Numerical and Driver will have to participate in the practical test in the respective subject and candidates who have cleared the practical test will be eligible to participate in the oral test.

Following are the rules/conditions and deadlines for filling the application form online:

(a) Interested candidates can fill the application form at https://cs.lakshmipur.gov.bd website. The form should be filled as per the instructions published on the website.

(b) Starting date and time for online application filling and examination fee submission from 22/04/2024 AD from 10:00 AM onwards. (c) Last date and time for submission of online application is 14/05/2024 AD upto 5:00 PM.

(d) Candidates who have received User ID within the said period can submit the examination fee through SMS within 72 (seventy two) hours from the time of online application submission.

In the online application form, the candidate has to scan his colored photograph (length 300 pixels × width 300 pixels) and signature (length 300 pixels × width 80 pixels) and upload it at the designated place. Photo size maximum 100 KB and signature size maximum 60 KB

Since the information filled in the online application form will be used in all the subsequent activities, the candidate himself will be hundred percent sure about the correctness of all the information filled before submitting the online application form.

Candidates shall keep a print copy (Colour) of the application filled online as an aid for any examination related needs and submit one copy at the time of oral examination.

Rules for Sending SMS and Payment of Test Fee:

After completing the online application form by properly filling the application form and uploading the photo and signature according to the instructions, the application preview with the photo can be seen on the computer.

After submitting the application form, the candidate will get a copy of the application with a user ID, photograph and signature. If there is any mistake in the Applicant’s Copy or unclear image (full black / full white / blurry) or the photo / signature is not correct then you can reapply.

However, after submitting the application fee, no further changes / modifications / enhancements are acceptable, so before submitting the application fee, the candidate must save his recent color photograph, correct information and signature in the Applicant’s Copy by downloading the PDF copy and printing it in color.

A User ID number will be given in the Applicant’s Copy and later using the User ID the candidate can send 02 (two) SMS through any Teletalk pre-paid mobile number for exam fee Tk 200 (two hundred) and Teletalk service charge 23/ – Total (Non-refundable) Tk.223/- (Two Hundred and Twenty Three) including Tk.72 (Seventy Two) hours.

It should be noted that even if all parts of the online application form are submitted and submitted, the online application form will not be accepted under any circumstances until the examination fee is submitted.

Receipt of admit card will be intimated on website (https://cs.lakshmipur.gov.bd) and through SMS on candidate’s mobile phone only to eligible candidates in due course. All communication related to the examination will be done on the mobile phone given by the candidate in the online application form, so keeping the said number active all the time, reading the SMS and following the received instructions immediately is prohibited.

After using the User ID and Password sent in the SMS, the candidate will download and print (color) the admit card containing the information such as roll number, post name, photo, exam date, time and center name etc. Candidates have to participate in this written exam.
